Band Booster Membership Fee (All adults eligible)
Regular Individual Membership$10.00Per Person
Whole Note Membership$100.00Per Family/Corporation
**Will be recognized in all band programs and website**
Half Note Membership$50.00Per Family/Corporation
**Will be recognized in all band programs and website**
Quarter Note Membership$25.00Per Family/Corporation
**Will be recognized on website**


Cave Spring Band Boosters is a 501(c)3 non-profit organization and cash donations are tax
deductible. (Please check with your tax advisor for guidance). Parents, please note: A
contribution here does NOT constitute your Student Fair Share. This form should be used only
for additional contributions to the band program.
